These are fillable Adobe PDF forms, you can fill them out in your web browser, on your desktop with Adobe Acrobat Reader DC or print and hand fill them in prior to our initial visit.

- Clicking on Download will open the form in a seperate tab in your browser.
- If your browser allows PDF forms to be filled out, enter the information and click the buttons at the end of the form to email the form back to us and/or print a hard copy.
- Firefox click on the download button and select 'Open With' and choose Adobe Acrobat Reader DC, fill out the form(s) and print and/or email it back to us.
- If you experience issues, you can manually email the form to reggie@aps.dog.
- Chrome allows filing out the form and printing only. To email and save, download to your computer and open with Adobe Acrobat Reader DC.
